<p class="wp-block-paragraph">A client approached us with a medical affiliate website. The project had been running for just over a year and operated in a highly competitive niche. Most of the traffic went to informational pages, and commercial queries barely made it into the top 10. This PBN case study focuses on how we changed that dynamic with a limited number of targeted placements.</p>

<p class="wp-block-paragraph">The referral domain dynamics for the two months prior to the request remained virtually unchanged. Growth was at a rate of 1-2 new domains per week, primarily from directories and crowdsourced platforms. The site&#8217;s backlink profile prevented it from competing with the top search results.</p>



<p class="wp-block-paragraph">While analyzing the project&#8217;s history, we noticed that before contacting us, the medical affiliate site had already shown some growth, but then declined: rankings dropped, and traffic declined. The reasons could have been varied, ranging from Google algorithm updates to seasonality and/or natural fluctuations in the niche. The client experienced the nadir of this decline in February and, at that point, decided to deploy five PBNs on key commercial pages, enlisting our services.</p>



<h3 class="wp-block-heading"><strong>The result of placement on our donors</strong></h3>



<p class="wp-block-paragraph">After 5 PBN backlinks were placed, we monitored their dynamics using standard SEO tools. We focused on two metrics: the number of referral domains and the distribution of search results.</p>



<p class="wp-block-paragraph">The screenshot below shows how the number of referring domains changed over three months. In February, the number was around 30-35. By mid-May, it had grown to 140-150. This doesn&#8217;t mean that all the growth was due to the 5 PBN links &#8211; some domains were acquired organically or as a result of other activities. However, the graph itself clearly shows that the client&#8217;s link profile has not stagnated and is growing.</p>

<p class="wp-block-paragraph">The second screenshot shows changes in rankings. While the number of keywords in the top 3 and top 10 was minimal in February, by the end of April the picture had changed. The number of positions in the 4-10 range increased significantly, while the share of keywords outside the top 50 decreased. This means that search engines began to perceive the site as more relevant to target queries.</p>

<p class="wp-block-paragraph">Organic traffic showed a strong upward trend, increasing from approximately 250 sessions per month to a stable 1,000–1,100. At the same time, the site&#8217;s visibility appeared to become more focused on relevant search queries, potentially indicating an improvement in overall traffic quality.</p>



<p class="wp-block-paragraph">The client considered these results sufficient for the initial phase. Although the five PBN backlinks were only one part of the broader SEO picture, they appeared to support the site&#8217;s upward momentum and contribute to improvements in rankings.</p>



<p class="wp-block-paragraph">Five links is a small number for the highly competitive medical SEO niche. However, even this limited placement yielded a significant increase in organic traffic and pushed commercial pages up the rankings.</p>



<p class="wp-block-paragraph">Why it worked. The donors were selected based on relevance and a clean history. The anchor list didn&#8217;t contain exact-match commercial keywords; branded, non-anchored, and generic phrases were used. The links led to various internal pages, not just the homepage.</p>



<h3 class="wp-block-heading"><strong>How would we consolidate the result?</strong></h3>



<p class="wp-block-paragraph">To strengthen your position and scale your success, we would suggest adding another 20 links, distributing them across three areas:</p>

<p class="wp-block-paragraph">At the same time, it&#8217;s worth expanding the anchor text list with LSI phrases and natural keywords like &#8220;learn more about the procedure,&#8221; &#8220;case studies,&#8221; and &#8220;patient reviews.&#8221; This will reduce the risk of spam and make the profile appear even more natural.</p>



<p class="wp-block-paragraph">The placements would be spread out over 8-10 weeks, since the gradual emergence of new donors seems more natural to Google than a sharp jump.</p>



<p class="wp-block-paragraph">With this approach, additional PBN placements could further strengthen rankings and improve the site&#8217;s ability to compete for higher positions.</p>



<h2 class="wp-block-heading"><strong>Risks, Google Penalty Concerns, and Safety Measures</strong></h2>



<p class="wp-block-paragraph">Any work with a PBN is associated with risks. Google explicitly states in its webmaster guidelines that links from private blog networks are considered a violation of its rules. In the YMYL niche, these risks are higher because the algorithms scrutinize such sites more thoroughly.</p>



<p class="wp-block-paragraph">We don&#8217;t claim that PBN is a secure tool. However, under certain conditions, the risks can be reduced to an acceptable level.</p>

<p class="wp-block-paragraph">The client posted only five links. This volume doesn&#8217;t create any anomalous activity that the algorithms could easily detect.</p>

This reduced the likelihood of Google identifying the link as purchased. Furthermore, links weren&#8217;t added all at once, but at intervals, to make the growth appear natural.</p>



<h3 class="wp-block-heading"><strong>What we recommend to clients when working with PBN</strong></h3>



<p class="wp-block-paragraph">Our experience shows that there are ways to avoid sanctions. For example:</p>
